Globally, 134 million adverse events contribute to 2.6 million deaths of patients each year due to unsafe care. The slogan for the day is “Speak Up for Patient Safety” and this year’s campaign will mobilize patients and their families, health workers, policymakers, academicians, researchers, professional networks and the healthcare industry to speak up. To tackle medical errors, we must have a health care system that make it easy to do the right thing and make it difficult to do the wrong thing.
The health system can be made more effective through establishment of clinical/operating guidelines, good data, good records, focused skill training and systematic audit of performance.
